We removed the Order Form

Hello.
We're very sorry to say that we've removed our Order Form.
The reason being is because everytime a shopper sends in their order form it somehow annoyingly ends up in our Junk mail!
Sometimes we don't even get our orders through as the mail is deleted at times.
